BASFI
Bath AS Functional Index

Ten patient-reported questions assessing the ability to perform daily functional tasks. The simple average of all ten responses gives the BASFI score on a 0-10 scale. Validated against physical measurements and correlates with long-term work disability in axial SpA.

2 to 4Mild LimitationSome functional restriction. Monitor for progression.
4 to 6Moderate LimitationMeaningful functional impairment. Associated with increased work disability risk.
> 6Severe LimitationSignificant disability. Review disease control and consider physiotherapy referral.

BASFI MCID: approximately 0.7 points. A change of this magnitude is considered clinically meaningful in the context of therapeutic intervention.

💡 Pearls and Pitfalls
BASFI captures the functional consequences of both inflammation and structural damage. Patients with long-standing radiographic damage may have persistently elevated BASFI even when inflammatory disease is well controlled. A high BASFI with well-controlled ASDAS-CRP suggests functional disability driven by structural changes rather than active inflammation.
BASFI may not differentiate peripheral from axial disease burden. In patients with significant peripheral arthritis (especially PsA), BASFI may reflect peripheral joint limitation rather than spinal/axial disease. For PsA functional assessment, the HAQ-DI is more validated.
🔬 Evidence

BASFI was developed by Calin, Garrett, and colleagues and published in the Journal of Rheumatology in 1994. The 10-item questionnaire was designed to assess function specifically relevant to axial spondyloarthritis. Validation demonstrated strong correlation with clinical measures and radiographic progression, and BASFI has been incorporated into the Bath indices as the standard functional outcome for AS/axial SpA.
